Appointment Request

Please use this form to request an appointment with us.

Once you've submitted this form, we'll call you to confirm the appointment, do a new client deposit if you're new to our practice, and get any additional details.

New clients:
• Please include in the Notes section where you've received care before so that the doctor can review your records ahead of your appointment. 
• If you can, please print out our new client form and fill it out ahead of your appointment.
• Please also note that we do require an exam deposit to make the first appointment as a new client. Subsequent visits do not need a deposit. The deposit goes as a credit on the account to be applied when you come in. If you need to cancel for any reason and don't want to reschedule, we can refund it as long as you give 24 hour's notice.
• We are currently seeing new clients on Mondays, Tuesdays, and Thursdays.

We are located on the corner of South Miami Boulevard and TW Alexander Drive.

We can't guarantee we will have the time and day you prefer, but we will do our best to work with your schedule.
